My experience has included number 1, 2 and 3 buy a new supervisor. My previous goals were not reviewed and my boss had no idea if my goals had been accomplished (she even stated she did not read my previous reviews - which had been exceedingly positive.)
She brought up "surprise problems" but could provide no examples. Her comments did not include a plan of what should be changed or worked on.
My boss is a long time co worker that was recently promoted (with my support!) Oh, the review was done the last possible day and at the end of the day so I had no opportunity to respond in any meaningful way.
I requested a meeting with her supervisor and met with HR.
I would say that my experience was a nightmare.
